We always accept new patients in our office! We are excited to meet you, and it is our passion to get to the root of your problem and help you achieve your goals.
To save time, please fill out your intake forms in advance. You can either bring them with you, or email them to: frontdesk@lifelonghealthchiropractic.com
No special attire is required for your new patient appointment. Please arrive 15 minutes early so we can process the necessary paperwork, and plan to be in the office for one hour during your initial consultation.

Scheduling an appointment is as easy as filling out the short form below to request a preferred day and time time or giving us a call at 612-354-2231.
We serve the neighborhoods of Northeast Minneapolis including the Arts District, St. Anthony, Northeast Park, Windom Park, Sheridan, Holland, Bottineau, and Marcy-Holmes.
Once you fill out the form, we will contact you to confirm a date and time, answer any questions and get you set for your initial visit.
